During your interview, we will discuss your goals and objectives for optimum oral health and the appearance of a “Smile of Your Dreams”.

Diagnostic Models
Periodontal Tissue Health Assessment
Oral Cancer Screening
Tooth Decay Assessment
Blood Pressure Screening
These procedures and records allow our doctors to assess your level of health before presenting you their findings and recommendations that will transform your smile. We anticipate serving you with the utmost care each time we see you.
Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hour notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
Please be aware that our staff will also call or text to confirm any upcoming appointments. If confirmation is not received, cancellation of the appointment is a possibility.
